ENROLLING IN SCHOOL

Please click here to download the Student Information packet to fill out and bring to registration. The following documentation must be presented at the time of enrollment:

• Legal Parent/Guardian driver's license

• Original Birth Certificate

• Current Immunization Record on GA Form 3231

• Current 4-point health screening on GA Form 3300

• Social Security Card

• 2 proofs of residency

  • one proof must be a utility bill

  • both proofs must list physical address (PO boxes are not allowed)
