Scoliosis Screening Information

Attention 6th and 8th Grade Parents!

On Friday, April 26, 2024, the Georgia Health Department will conduct Scoliosis Screenings for all 6th and 8th grade students. An information letter and the scoliosis screening form were sent home with your student earlier this week. You have three options:

1. Opt your student out by having his or her doctor complete Form #4400 by April 22, 2024.

2. Opt your student out by completing the top portion of Form #4400 and selecting opt-out at the bottom of the page with your signature by April 22, 2024.

3. Have your child participate in the health department's scoliosis screening event.
